Community Intervention Team

The purpose of CIT is to free up beds/trolleys in SJH,and facilitate clients early discharge home. Also, to prevent re-admissions to hospital, by implementing nursing care in the home “out of hours”. 

When dispatched, two team members visit the client in their own home, that evening, if requested. A full nursing assessment is done, and the nurse decides on further visit needs. We also have an on-site wound clinic (Rathmines) for clients who do not want a home visit.

Hours of duty:

7 Day nursing service in the Dublin area
(Last referral at 10pm) 

Criteria for acceptance:

• over 18yrs
• Client must reside within Local Health Areas of Dublin West, Dublin South West, Dublin South city & Dublin South East.

 Interventions include:

All nursing care in the community including the following:

• PICC/HICC line flushing & site dressings
• COPD care including O2 saturation monitoring
• Supra-pubic re catheterisation
• IV medications

 Who can refer:

• G.P.’s
• Health care professionals from SJH
• Community Nursing team.

• All referrals received will be confirmed with referrer

 Staff involved are Joan Bourke, Asst Dir Nursing, and a dedicated nursing and carer team.
