Community Intervention Team

The South Dublin Community Intervention Team is a specialist nurse led health professional team which provides a rapid and integrated response to patients with acute episodes of illness who require enhanced services or acute intervention for a defined short period of time. This care is provided either in the patient’s home, residential setting, or in the community as deemed appropriate. This avoids acute hospital attendance, admission or facilitates an early discharge for the referred patient.


Referrals are accepted from 9.30am to 21.30pm

  • IV Antibiotic Administration
  • PICC/HICC/PORT Care including Chemo Pump Disconnections
  • Intramuscular & Subcutaneous Injections
  • COPD Early Discharge Assessments
  • INR Monitoring with Heparin Bridging
  • Male/Female Suprapubic & Urinary Re-Catheterisation
  • Sub-Cutaneous Fluid Administration
  • Phlebotomy
  • Palliative Care
  • Evening Support Calls
