The Integrated Care Programme for Older Persons (ICPOP) at St. James’s Hospital is a specialist service designed to support frail older adults within the community and provide alternatives to acute hospital admission. It is a joint initiative between the hospital and the HSE. Using interdisciplinary teams—including nurses, therapists, and geriatricians—it provides comprehensive, community-based care to avoid unnecessary hospital visits. It focuses on frailty, falls, and dementia management.

BY OUR INTEGRATED CARE TEAM

•  Our team of doctors, nurses, physiotherapists, occupational therapist and health care support assistants work with you to keep you well at home.
•    Once referred to us the Multidisciplinary Team will do a comprehensive clinical assessment on you to determine a diagnosis.
•    Our team will support you and your family post diagnosis to develop a personalised care and support plan.
•    Together this team works with St James Hospital and primary care services at a specific period of time in your like where you may require our support for a period of about 6 weeks. In this 6-week period we can offer you a programme of care that is specific to your needs. This can vary from identifying achievable rehab goals that you as independent as possible, to liaising with the specialist services in the community to support your in accessing the resources you need to stay at home.
