Cardiac Rehabilitation

GP Information

Patients are visited on the wards and CCU (Phase I) by one of the cardiac rehab nurse co-ordinators. Advice on discharge and risk factors for heart disease is given. The overall concept of cardiac rehabilitation is explained.

Patients’ are given an appointment for phase II OPD. At this meeting patients are again assessed for their suitability for the programme. A date for starting the programme is given to the patient and an appointment for an Exercise Stress Test. Advice on lifestyle changes is given and any relevant referrals are made at this appointment.

Phase III cardiac rehabilitation is the six or eight week programme of exercise and education.

Phase IV involves maintenance of lifestyle changes. At St James’ we invite participants of programmes back after six months for a recap of advice already given on the programme. A “mindfulness” stress management course is also provided as part of Phase IV cardiac rehabilitation.
