Referral Procedures
To obtain an appointment with the multi-disciplinary team we require a referral from your GP or Physician, this should be sent to the address above. All referrals must clearly state the patients name, address, date of birth and contact details, as well as a brief summary of the patient’s condition. These referrals will be prioritised and an appointment will be forwarded accordingly.
