Referral Procedure

Patients living in the catchment area may be referred to our service by a G.P. all patients require a medical referral. Referral usually takes the form of a detailed letter and the letter will be discussed by the relevant multidisplinary team where upon an appropriate assessment and care plan procedure is initiated. Occasionally, where appropriate, a patient's initial assessment may take place in their home. In acute emergencies, a patient may be referred to the Emergency Department where a 24 hour on call service is provided. Referral letters should be sent to:
